Please Note the following:
MSYSA uses both sides of the USYS cards
Players and/or Coaches and/or Managers must sign both halves of the card before taking the card to registrar for processing. Please draw a line above the FIFA FAIR PLAY, this will show the player or adult where to sign the card.
A current color photo must be attached to both halves of the card. The proper placement of the photos is shown below.
Please note that the player cards should be signed in cursive (possible exception for the U9) and using blue or black ink.
